CPRA Request Form

The California Privacy Rights Act of 2020 (“CPRA”) grants individual California residents limited data privacy rights that include “request to know”, “request for access', “request to delete”, and “request to correct”.  You must fully complete this form to exercise your request rights. After we have received your request and have verified your identity, your request will be processed within the time period mandated by the CPRA.
